These include 81 long-term acute care hospitals (LTAC), 19 inpatient rehabilitation facilities (IRFs), 19 sub-acute units, 614 home health units, and 102 inpatient rehabilitation units.

The aging population in the United States is steadily increasing the demand for post-acute services. According to the company, nearly half of Medicare patients leaving acute care require post-acute care. However, most post-acute care is generally not well managed and suffers from high readmission rates. Therefore, both hospitals and insurance companies are looking for improvements to post-acute care.
