Accepted across both Cardio-Kidney-Metabolic tracks, Innovaccer brings AI-led care orchestration to Medicare's most significant chronic care payment reform ahead of July 2026
Innovaccer, a leading healthcare AI company, today announced that the Centers for Medicare & Medicaid Services (CMS) has accepted its application under Story Health Partners for the Advancing Chronic Care with Effective, Scalable Solutions (ACCESS) Model, positioning the company for participation at the program's July 2026 launch. Innovaccer has been accepted across both Early Cardio-Kidney-Metabolic (eCKM) and Cardio-Kidney-Metabolic (CKM) tracks, spanning hypertension, diabetes, chronic kidney disease (CKD) and cardiovascular disease conditions affecting more than two-thirds of all Medicare beneficiaries.
The CMS ACCESS Model establishes an outcome-aligned payment framework for technology-enabled chronic care precisely the operating model Innovaccer's platform is built to power.
"For a decade, Innovaccer has partnered with health systems on some of the most complex chronic care challenges in Medicare. We have built the AI infrastructure, the workflows, and the clinical programs to manage these populations at scale. The ACCESS Model represents Medicare's commitment to paying for outcomes rather than activity, and that is precisely the operating model Innovaccer was designed to support. We look forward to delivering measurable results for our health system and provider partners in the first cohort," said Abhinav Shashank, Cofounder and CEO, Innovaccer
Innovaccer's ACCESS participation model is designed to be zero-risk for health system partners. Organizations continue billing Medicare fee-for-service without disruption, while Innovaccer delivers technology-enabled care, and takes on outcome risk directly for the specified chronic conditions. CMS pays Innovaccer; health systems earn new referral and co-management revenue without absorbing new infrastructure or compliance obligations. For ACO participants, the model is complementary by design as ACCESS patients remain attributed to the health system's ACO, and program spend is excluded from ACO benchmarks for the first 18 months entirely.
Innovaccer also plans to expand into the Musculoskeletal and Behavioral Health tracks, positioning itself as a comprehensive chronic care management solution across all four ACCESS clinical tracks.
At the core of Innovaccer's ACCESS model is its end-to-end AI care orchestration platform, built to run the complete patient lifecycle without adding operational burden to clinical teams. It continuously identifies and risk-stratifies eligible Medicare populations, activates them through automated SMS, IVR, and email outreach, and enables providers to enroll patients directly from existing workflows. From there, Innovaccer manages everything: program orientation, care plan goal-setting, baseline CMS submissions, PCP coordination, remote monitoring, medication optimization, and triage risk alerts, while returning real-time clinical visibility to the care team at every step.
Central to Innovaccer's differentiation in the ACCESS Model is Adaptive Program Intelligence, Innovaccer's proprietary engine that dynamically adjusts intervention intensity, from AI-only, hybrid, or high-touch human care based on each patient's engagement and response patterns in real time.
Throughout, patient relationships remain with the health system. All in-person care is referred back to the health system's provider network, and real-time clinical data flows back to care teams, ensuring continuity of care and protecting long-term patient attribution.
With the first cohort launch approaching, Innovaccer is onboarding health system partners now and can have eligible populations identified within weeks. Interested health systems and providers can learn more https://innovaccer.com/cms-access
